• Phone Number
• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Using your device, go to this website: https://support.fubo.tv/hc/en-us
2. Click on the large “?” button in the bottom right corner
3. Put your information into the page where it’s requested of you
4. Let the rep know that you’re calling to cancel
5. Request proof of your transaction in the form of an email or confirmation number
6. As usual, just in case, keep the proof that you’ve cancelled handy.
