• State/Province/Region
• ZIP/Postal Code
• Country (if outside USA)
• Email Address
• The phone number and company name from the bottle

Follow these steps:

1. Contact customer service at the phone number on your bottle
2. Ask for information about cancelling your account
3. Provide the agent with your account info
4. Let the rep know that you’re calling to cancel
5. Make sure that you won’t be charged again by asking the rep
6. Request that you be refunded
7. Request proof of your transaction in the form of an email or confirmation number
8. As usual, just in case, keep the proof that you’ve cancelled handy.
